The District Governor’s Citation for 2011-2012 recognizes and honors outstanding Clubs within the District who not only receive a 2011-2012 Presidential Citation, but also specifically contribute to accomplishment of the 2011-2012 District Goals.

Please send the 2011-2012 District Governor’s Citation request form (see below or click here to download a copy), signed by the Club President and appropriate Assistant District Governor, to the District Governor by March 31, 2012, in order to receive recognition at the District Conference.

Requests received after March 31, but not later than June 30, 2012, will be processed on an "as received" basis, with presentations of Citations made at a Club meeting.

Submission of supporting materials with the request is not required.

2011-2012 District Governor’s Citation

To qualify for this Citation, a Club must (1) have applied to receive or have received either a 2011- 2012 Presidential Citation; (2) meet the requirements contained in Part 2 of the request form; and (3) complete the number of activities indicated for this Citation for each of the Subparts in Part 3 of the request form.

PART 1

_____ Pre-requisite - Presidential Citation (box must be checked):

_____The Club has applied to receive or has received a 2011-2012 Presidential Citation (qualifies Club for District Governor’s Citation).

PART 2

_____ Requirement - President-elect training (box must be checked)

___ President-elect has attended/will attend Pre-PETS, PETS and District Assembly in 2012.

_____ Requirement - Club Administration (at least three boxes must be checked)

___ Incoming officer information was entered on line by December 31, 2011.

___ Attendance reports for Rotary year 2011-2012 for July through the month previous to the date of this recognition request were submitted in accordance with District procedures and received no later than the 15th of the following month for the preceding month.

___ Club dues and other fees payable to RI were paid within 30 days of the receipt of the semi-annual report packets in July and January.

___ Club dues and other fees payable to the District were paid within thirty days of the date of receipt of the invoice from the District.

PART 3

Membership

___ District Governor’s Citation - have a net increase in Membership – baseline is the number of Club Members as of July 1, 2011.

___ District Governor’s Citation - achieve an 85% retention rate of existing Members.

Rotary Foundation

_____ Completion of three (3) required for the District Governor’s Citation.

___ Achieved STAR Club status.

___ Qualified for a Globe Club Award.

___ Qualified for Polio Eradicator Club Recognition.

___ Qualified/will qualify for Every Rotarian, Every Year Club recognition.

___ Qualified/will qualify for 100% Rotary Foundation Sustaining Member Club recognition

New Generations Programs

_____ Completion of two (2) required for the District Governor’s Citation.

___ Club sponsors a Rotaract Club.

___ Club sponsors an Interact Club.

___ Sponsored a RYLA participant.

___ Endorsed an Interact Club Member for a James L. Bomar, Jr., Interact Scholarship. Award and /or the Fred Brown Interact Award of Merit.

___ Sponsored a new Interact or Rotaract Club.

___ Participated in a joint project with an Interact or Rotaract Club.

___ Sponsored Interact Club is represented at the annual Interact Conference.

Education and Leadership

_____ Completion of three (3) required for the District Governor’s Citation

___ A Club Member has graduated from the District Leadership Academy and/or is attending the District Leadership Academy.

___ A Club Member is attending/has graduated from the Rotary Leadership Institute.

___ A Club Member serves as a District officer or an Assistant Governor, or serves on a District-level committee.

___ A Club Member has attended/will attend the 2012 District conference or RI Convention. 4

___ The Club Secretary and/or the Club Treasurer attended/will attend the 2012 District Assembly.

___ The Club Committee Chairs and/or their representatives for at least three of the four following areas, Membership, New Generations, Foundation, and Public Relations, attended an Annual District Educational Seminar during 2011.
